When you acquire gear in a entirely random fashion, with random attributes or appearances, ranging from talking to animals to 'stains', it will have a specific size. Just like you would see in a store, there are many sizes for different types of clothing and armor, and wearing one too large or too small is going to cause problems.
On top of that, you'll be experiencing your body transformation in ways that your clothes aren't accustomed to, either causing them to rip, to make your feel uncomfortable and irritated, or provoke some reactions from either yourself or those around you.
Gear gives you new ways to do what you already know how to do, or changes how you have to do something. It's not a static boost, but a new way to play. Not all gear is this way, but the key is to find gear that fits the current situation - how you look and how you want to play.
Keep in mid, you can only get benefits for each slot ONCE. An additional note is that while there seem to be a lot of options, some of them cannot be worn at the same time. No one would wear a shirt and a dress and a bra and a bustier at the same time with jeans, thigh-high boots, a helmet with a hooded cloak. It's reasonable to wear reasonable things!

* = Typically things come in sets that, to get the full effect, you need to wear all of the components. Sometimes you need to only wear one, and other times you might want to mix and match, so for each of these categories you are only allowed a maximum of 2 items to prevent anything game breaking and overpowered. There are also In Character reasons for this that will be explained if (and when) it happens!
